First aid log template (PDF) to print

Record sheet for first aid with all mandatory information – as single sheets so that entries remain confidential. With notes on retention and data protection.

As of PDFAMS Go editorial team

Every first aid treatment in a German company must be documented (Section 24 (6) DGUV Regulation 1). The record proves that an injury occurred at work – important if a small wound later turns into an infection or an insurance claim.

The classic first aid log as a bound book has a catch: everyone who makes an entry sees the others' entries. That is why this template works with single sheets that are kept locked away.

What the template contains

  • PDF sheet with all mandatory information: date, time, injured person, what happened, injury, measures, first aider, witnesses
  • Eight entries per sheet, sheet number for reference
  • Notes on retention, confidentiality and accident reports directly on the sheet

How to use the template

  1. 1

    Keep sheets ready

Keep sheets at every first aid kit or with the first aiders. Completed sheets go into a locked folder.

  2. 2

    Record immediately

Make an entry after every first aid treatment – ideally by the first aider, with witnesses. Even a plaster for a cut counts.

  3. 3

    Keep for five years

Keep the sheets for five years and make them accessible only to authorised persons. If someone is unable to work for more than three days, also submit the accident report.

Document management in AMS GoFile records such as first aid log entries, accident reports and protocols with access rights so that only authorised persons can see them.

Legal basis

  • Section 24 (6) DGUV Regulation 1: documentation of first aid, retention for five years
  • DGUV Information 204-020: first aid log
  • Art. 5 and 9 GDPR: confidentiality of health data
  • Section 193 SGB VII: accident report if someone is unable to work for more than three days

Frequently asked questions

May the first aid log be kept digitally?

Yes. DGUV Regulation 1 requires records, not a particular form. A digital solution must capture the information completely, restrict access to authorised persons and keep the data available for five years.

Who may see the first aid log?

Only persons who need the data for their task – such as first aiders for making entries, the company doctor and the persons who prepare an accident report. It is health data, which is specially protected.

Do I have to record every small cut?

Yes. Small injuries in particular can become inflamed or cause problems later. The entry then proves the connection with work – for the injured person, that is the most important benefit of the first aid log.

You may use and adapt the template freely in your own company and for your own clients. It reflects the regulations named as they stand, but does not replace legal advice or adapting it to your own company.
