DGUV V3 test report template (PDF) and test list (Excel)

Report for the repeat test of electrical equipment with visual inspection, measured values for protective conductor resistance, insulation resistance and leakage current, functional test and next test date – plus an Excel list for many devices.

As of PDF and ExcelAMS Go editorial team

Under DGUV Regulation 3, portable electrical equipment must be tested at regular intervals – the guideline is every six months, in offices with a low failure rate up to two years. Every test needs a report with visual inspection, measured values and functional test; the sticker alone is not enough.

The PDF report is intended for testing individual devices, the Excel test list for many devices in one round. The second worksheet helps you calculate the failure rate – the basis for extending test intervals with good reason.

What the template contains

  • PDF test report with visual inspection, measured values with limit values according to DIN EN 50699, functional test and result
  • Fields for protection class, test device, next test date and sticker
  • Excel test list for up to 60 devices per round with drop-down lists
  • Worksheet for calculating the failure rate per test round

How to use the template

  1. 1

    Clarify tester and test device

Tests may be carried out by a qualified electrician or an electrically instructed person under their supervision – with a suitable, calibrated test device.

  2. 2

    Inspect, measure, test

First the visual inspection, then the measurements depending on the protection class, and finally the functional test. Lock away and label failed devices immediately.

  3. 3

Document and analyse the failure rate

File the report and transfer the next date to the inspection register. Calculate the failure rate after each round – it determines the test interval.

Inspection register in AMS GoImport measured values from the test device, file reports per item of equipment and get automatic reminders of the next test.

Legal basis

  • DGUV Regulation 3, Section 5: testing of electrical installations and equipment
  • Section 14 of the Ordinance on Industrial Safety and Health (BetrSichV) and TRBS 1201: inspection of work equipment
  • DIN EN 50699 (VDE 0702): repeat tests of electrical equipment
  • Section 17 BetrSichV: retention of records

Frequently asked questions

Which measurements are part of the DGUV V3 test?

Depending on the protection class, the protective conductor resistance (protection class I), the insulation resistance (all protection classes) and the protective conductor or touch current. DIN EN 50699 sets the limit values; they are shown next to each measurement field in the template.

May an instructed person sign the report?

The electrically instructed person documents their test and signs as the tester. Responsibility for the test procedure and the assessment lies with the qualified electrician under whose direction and supervision the test is carried out – which is why the template has a second signature field.

How do I calculate the failure rate?

The number of failed devices divided by the number of all devices tested in one round. If it is below 2 per cent, the test interval may be extended – in offices to a maximum of two years, otherwise to a maximum of one year.

You may use and adapt the template freely in your own company and for your own clients. It reflects the regulations named as they stand, but does not replace legal advice or adapting it to your own company.
